How To Access Services

There are several ways to access and/or apply for services (see options below) at the Chanda Center for Health, at our Lakewood facility through our disability-competent providers and community partners. Private Pay

The Chanda Center for Health provides self-pay services for able-bodied individuals and those with long-term disabilities who have the resources to pay at our standard rates. As our way to say thank you, caregivers, service members, first responders, and seniors receive $20 off.

Workers’ Compensation

The Chanda Center for Health is a provider of workers’ compensation under Pinnacol. However, if you have worker’s compensation with another provider, please talk with your worker’s comp case manager to determine possible coverage of care.

Complementary & Integrative Health Waiver

Complementary & Integrative Health (CIH) waiver reimbursement through Medicaid
Formally know as the Spinal Cord Injury (SCI) Waiver

To qualify for the CIH waiver, you must meet the following requirements:

  • Be 18 years of age or older
  • Have a diagnosed spinal cord injury (traumatic and nontraumatic), multiple sclerosis, brain injury, spina bifida, muscular dystrophy, or cerebral palsy.
  • Inability for independent ambulation resulting from one of these conditions.
  • Be financially eligible

Your Medicaid case manager will help enroll in the waiver. Case management agencies are based on the county where you live. The care coordination team at the Chanda Center for Health is available to help find your case manager information if unknown and answer your questions. We also provide massage under other Medicaid waivers including the Children with Complex Health Needs Waiver (CwCHN), the Children’s Extensive Support (CES) waiver and the Supported Living Services (SLS) waiver. Receive Access to Discounted Services

The Chanda Center for Health provides access to integrative treatments on-site at its Lakewood facility. To qualify for discounted services you must meet the following requirements:

  •  Living with Spinal Cord Injuries, Cerebral Palsy, Multiple Sclerosis, Spina Bifida, Brain Injuries, Muscular Dystrophy or other neurological conditions.
  • Inability for independent ambulation.
  • Do not have medical insurance that covers the requested therapy type
  • Inability to pay for services based on financial restrictions
  • Meets Qualifications for a Medicaid Waiver program.
