How To Apply & Receive Services

There are several ways to access and/or apply for discounted services at the Chanda Center for Health. The Center provides access to integrative treatments on-site at its Lakewood facility as well as at off-site facilities along the front range and across the nation through our network of providers.

If you are already enrolled in Health First Colorado (Colorado’s Medicaid Program), contact your case worker or local Single Entry Point (SEP) agency to start the waiver application process. Call or email the Center for more information and to make an appointment to get help accessing your case manager. 1-800-766-4255 ext. 1

Spinal Cord Injury Waiver

Spinal Cord Injury (SCI) waiver reimbursement through Medicaid

To qualify for the SCI waiver, you must meet the following requirements:

  • Be 18 years of age or older
  • Have a diagnosed spinal cord injury

  • Have significant functional impairment
  • Be financially eligible
  • Live in Denver, Adams, Arapahoe, Douglas or Jefferson County

Receive Access to Discounted or Donated Services

The Chanda Center for Health provides access to integrative treatments on-site at its Lakewood facility as well as at off-site facilities along the front range and across the nation through our network of providers. To qualify for discounted or donated services you must meet the following requirements:

  • Have a physical disability due to a spinal cord injury, brain injury, Cerebral Palsy, Multiple Sclerosis or Spina Bifida
  • Use a wheelchair 100% of the time outside of the home
  • Do not have medical insurance that covers the requested therapy type
  • Inability to pay for services based on financial restrictions

Apply for On-Site Financial Assistance

Apply for Off-Site Financial Assistance

*Due to COVID-19, we no longer have funding available for off-site services in 2021. If you have questions about applying for on-site or off-site services, please call the Center at 1-800-766-4255.

Apply for Intensive Fund Services

The intensive fund provides off-site services funding for individuals who are newly injured to access adaptive exercise. Applications will be accepted for this program in 2021.

Self Pay Services

The Chanda Center for Health provides self-pay services for able-bodied individuals and those with long-term disabilities who have the resources to pay.

Chanda Center for Health rates:

  • Acupuncture (1-hour session): $70
  • Massage (1-hour session): $70
  • Chiropractic (30-minute session): $55
  • Counseling Services (50-minute session): $70
  • One-on-one Yoga Session (1-hour session): $70
  • Adaptive Yoga Class: Free for those with physical disabilities
  • Care Coordination Services: Free upon enrollment into one or more of the services above

Workers’ Compensation

The Chanda Center for Health is a provider of workers’ compensation under Pinnacol. However, if you have worker’s compensation with another provider, please talk with your worker’s comp case manager to determine possible coverage of care.

TriWest Healthcare Alliance

We are an approved acupuncture provider through the Department of Veterans Affairs (http://www.triwest.com/en/).

Other Medicaid Benefits

The Chanda Center for Health strives to make all services available and financially accessible for those on Medicaid.

The Center is recognized as a behavioral health and physical therapy provider through Medicaid.

We also provide massage under other Medicaid waivers including the Children with Life Limiting Illness (CLLI) waiver, the Children’s Extensive Support (CES) waiver and the Supported Living Services (SLS) waiver.
