How To Access Services

There are several ways to access and/or apply for discounted services at the Chanda Center for Health. The Center provides access to integrative treatments at its Lakewood facility through our disability competent providers and community partners.

How To Apply & Receive Services

If you are already enrolled in Health First Colorado (Colorado’s Medicaid Program), contact your case worker or local Single Entry Point (SEP) agency to start the waiver application process. Call or email the Center for more information:

Complementary & Integrative Health Waiver

Complementary & Integrative Health (CIH) waiver reimbursement through Medicaid
Formally know as the Spinal Cord Injury (SCI) Waiver

To qualify for the CIH waiver, you must meet the following requirements:

  • Be 18 years of age or older
  • Have a diagnosed spinal cord injury (traumatic and nontraumatic), multiple sclerosis, brain injury, spina bifida, muscular dystrophy, or cerebral
    palsy.
  • Inability for independent ambulation resulting from one of these conditions.
  • Be financially eligible

Your SEP agency is based on the county where you live.

  • Rocky Mountain Human Services is the SEP for Adams, Arapahoe, Denver, Douglas and Elbert counties

Receive Access to Discounted or Donated Services

The Chanda Center for Health provides access to integrative treatments on-site at its Lakewood facility. To qualify for discounted services you must meet the following requirements:

  • Have a physical disability due to a spinal cord injury, brain injury, Cerebral Palsy, Multiple Sclerosis or Spina Bifida
  • Inability for independent ambulation.
  • Do not have medical insurance that covers the requested therapy type
  • Inability to pay for services based on financial restrictions

Other Medicaid Benefits

The Chanda Center for Health strives to make all services available and financially accessible for those on Medicaid.

The Center is recognized as a behavioral health and physical therapy provider through Medicaid.

We also provide massage under other Medicaid waivers including the Children with Life Limiting Illness (CLLI) waiver, the Children’s Extensive Support (CES) waiver and the Supported Living Services (SLS) waiver. Workers’ Compensation

The Chanda Center for Health is a provider of workers’ compensation under Pinnacol. However, if you have worker’s compensation with another provider, please talk with your worker’s comp case manager to determine possible coverage of care.

The Chanda Center for Health provides self-pay services for able-bodied individuals and those with long-term disabilities who have the resources to payat our standard rates:
